Description | In this course, students will learn the steps involved in conducting incident / accident investigations. Students will learn the basics of the Canadian Standards Association (CSA) Z1005-17: Incident Investigation guidelines on how to conduct accident investigations. Students will review major disasters to learn about factors leading to the accidents. They will analyze human factors, materials, work environment, task and process involved leading to the accident. They will also explore the underlying factors leading to failure of workplace safety systems and apply relevant controls to prevent the recurrence of an accident. The use of various incident causation models, such as Reason’s Swiss Cheese model will be explored by students to analyze incidents. Students will learn to analyze the data gathered to determine findings and make recommendations. Finally, they will learn to use interview and other person-to-person techniques such as mediation effectively to resolve conflicts. |
